docs: update unified-resource-architecture with migration status

- Documented WebSocket state migration as completed
- Listed all pages migrated to unified resources
- Outlined future Phase 6 cleanup tasks
- Clarified the strategic shift from dedicated /resources view to
  migrating existing pages
