test: Add tests for normalizeEndpointHost, SelectInterval, generateDockerHostIdentifier
- normalizeEndpointHost: port-only URL host (parsed.Host fallback), edge cases for URL parsing (94.4% -> 100%) - SelectInterval: negative penalty, instance key derivation, clamping edge cases (96% - remaining is unreachable dead code guards) - generateDockerHostIdentifier: empty base fallbacks, suffix candidates, hash suffix, numeric suffix increments (91.7% -> 100%)
